This Plan, which will pay appropriate usual and customary charges for routine services and supplies that are authorized by a dentist, including —

• 100 percent of preventive services.
  
• 80 percent of restorative services including extractions, fillings, oral surgery, endodontics.
  
• 50 percent of major services including crowns, bridges, dentures, and surgical periodontics.
  
• No deductible on covered procedures.
  
• Annual maximum benefit of $2,000/person.
  
• 50 percent of orthodontia expenses for dependent children under age 19.
  
• A separate lifetime orthodontia maximum of $3,000.
  
• Log on to www.deltadental.com to search for an in-network dentist in your area. Keep in mind that out-of-network dentists are covered with the same benefits listed above, but in-network discounts are not applied. No deductible on covered procedures.
