This Plan, which will pay appropriate usual and customary charges for routine services and supplies that are authorized by a dentist, features —

• No deductible on covered procedures.
• 100 percent of preventive services.
• 50 percent of restorative services.
• 50 percent of prosthodontic services.
• Annual maximum benefit $2,000/person.
• 50 percent of orthodontia expenses up to age 19. A separate lifetime maximum of $3,000/person.
