Finding Blue Cross/Blue Shield Providers The Brethren Medical Plan uses exclusively the Blue Cross/Blue Shield affiliated networks. Go to www.bcbs.com to find in-network physicians and facilities in your region.
When you get to that site, along the left side of that web page are some other links, click on the "Blue Card Hospital & Doctor Finder" link, which can be found in the left column, second from the bottom.

Under the section, "Help us narrow the search," it asks for an Alpha Prefix from your ID card: type CQM in the box and then click on "Continue" at the bottom.

Under "Search Center," you have a few options on how to search for a provider.

Enter a zip code if you are looking in general. Click on the radius in which you want to drive to a doctor/facility. Click "Search by Specialty" and then click on the specialty in which you are searching. This search will give you all the network providers based on the criteria you selected.

If you wish to broaden the search, go back and click on a larger radius OR enter a specific provider's name.
